Cause and effect diagrams, also called Ishikawa diagrams or fishbone diagrams, illustrate how various factors might be linked to potential problems or effects. Figure 8-8 is an example of a cause and effect diagram. A possible root cause can be uncovered by continuing to ask "why" or "how" along one of the lines. "Why-Why" and "How-How" diagrams may be used in root cause analysis.
